Long Term Care is an optional benefit program that provides insurance coverage to pay some or all of the costs of assisted living when a person is unable to take care of their daily living needs. Covered services can be received in:
- Nursing Facility
- Assisted Living Facility
- Hospice Program
- Adult Day Care
- Home Health Care Agency
Participants can continue their plan when they retire or leave state service.
Enrollment Eligibility: In addition to yourself, your family members may also be able to apply for benefits.
- Retirees
- Spouses
- Parents (incl. In laws)
- Children (incl. In-laws, adopted & step)
- Grandparents (incl. In-laws)
- Siblings (incl. In-laws, adopted & step)
Benefit Eligibility: To be eligible for Benefits, periodic proof from a Licensed Health Care Practitioner that:
- You need substantial assistance with a least two of the Activities of Daily Living for a period expected to last at least 90 days; or
- You need substantial supervision to protect you from threats to health and safety due to severe cognitive impairment. Activities of daily living include bathing, continence, dressing, eating, toileting, and transferring.
