Nominate Employee of the Month

Completed nomination forms must be received in HR/EO no later than the first day of September, December, March or June for that nominee to be eligible for selection in the following quarter. Once a nomination form has been received, the nominee will be eligible for selection for the next two consecutive quarters. A nominee must have a half-time or greater appointment in a permanent, continuing position at the time of nomination and have completed one (1) year of service. Assistance in completion of a nomination form can be received by calling Human Resources and Equal Opportunity at 864-7408. Normally, the Recognition Committee will not conduct interviews. The nomination form will be the primary source of information in the selection process. Supporting information is very helpful and may be sent along with this form.
